Human tasks randomly remaining open after a boundary event

Symptoms
A human task activity has a timer boundary event. Randomly the tasks will not get interrupted by the boundary event; instead they'll remain open (despite the instance moving forward in the flow). In the logs you'll see the following exception at that time:
