Account status Going To Waiting and No Process Task is Triggered.

Symptoms
Using OIM and provisioning an account for example: an EBS account and the account goes to Waiting status instead of getting provisioned.
No process tasks including System Validation is triggered .
The issue can be reproduced with the following steps:
1. Login in to Identity console
2. Go to Users, select a user
3. Request an EBS account and submit it
Log file shows the following :
