Request Is Not Displayed in a User's Pending Approvals

You've observed that although a task is assigned properly, the approver assigned the request is not able to see it in Pending Approvals. However, if the user instead displays the Inbox, he can see the request there.
This can occur during development when deploying/undeploying a number of custom composites. Or after you've modified the view as described in the following note:
