FRM-93652 when Login to Oracle Forms Application after Enabling OAM Password Policy (Doc ID 2368130.1)

Symptoms

Oracle Forms is configured with OAM and SSO.  You disable password policy in OID and enable password policy on OAM per instructions in:
  <Note:1626347.1> How to configure OAM11GR2 Password Policy   

After OAM password policy is set up successfully, you login to any Oracle Forms application and it immediately crashes with error:

FRM-93652 The runtime process has terminated abnormally

For Example:

It consistently crashes after OAM login.  Oracle Forms was working fine before using OAM password policy.
