WebLogic 12.2.1.3 IIS Plugin is not Passing Requests to WebLogic Managed Server with 404 Errors (Doc ID 2369978.1)

Symptoms

Using WebLogic IIS Plugin version 12.2.1.3.0.
When attempting to use the plugin to proxy APEX/ORDS requests to WebLogic Managed Server, the following 404 File Not Found error occurs when testing a connection.
No log entries exist for the <URI> requests in the WebLogic Plugin Log.

IIS Logs show the following requests are received. (eg: <DRIVE>:\inetpub\logs\LogFiles\value\u_exvalue.log)

Changes

A Virtual Directory was created within IIS for <URI>.
