AD Connector Account Provisioned Via Access Policy Stays in Provisioning Status Due to Missing Required Data

Access policy based provisioning for AD account is going to "Provisioning" status instead of "Provisioned" status in OIM and Account is not getting provisioned in the target system (AD). The system validation task is pending status.
