OUD 11g/12c - Error cn="LDAPS Connection Handler,cn=Connection Handlers,cn=config has experienced consecutive failures while trying to accept client connections: Too many open files" (Doc ID 2389307.1)

Symptoms

The directory server stops accepting new connections and reports the following error:

cn=LDAPS Connection Handler,cn=Connection Handlers,cn=config has experienced consecutive failures while trying to accept client connections: Too many open files (ServerSocketChannelImpl.java:-2 ServerSocketChannelImpl.java:250 LDAPConnectionHandler.java:1006). This connection handler will be disabled

category=PROTOCOL severity=NOTICE msgID=2556181 msg=Stopped listening for new connections on LDAP Connection Handler 0.0.0.0 port <PORT>

 
Other errors found when issue occurs -

server.out Error -

category=RUNTIME_INFORMATION severity=NOTICE msgID=20381721 msg=The maximum number of file descriptors allocated to the server process: 4096

OUDSM/ODSM Error -

Could not connect to server <FQDN>:<ADMIN_PORT>. Check that the server is running and that it is accessible from the host where Oracle Unified Directory Services Manager is installed. Details: <FQDN>:<ADMIN_PORT> - Connection refused (Connection refused)
