Audit Trail Icons not displayed in Enterprise Manager

Symptoms

The customer received an error in Enterprise Manager (EM) when trying to access a BPM Instance Flow (or Audit Trail) and the icons (or images) for the activities were not displayed. The diagnostic logs recorded this error:
