12c Forms and Reports Configuration JDBC Component Schema Screen Fails with "CFGFWK-64217: JDBC Data Source Validation Failed" (Doc ID 2411459.1)

Symptoms

When attempting to configure Forms and Reports 12.2.1.3, the JDBC Component Schema step for the Component Datasources fails with the following errors:


The Configuration Wizard for the JDBC Component Schema step shows the following: 

Changes

The OPSS Audit Schema, OPSS Audit Viewer Schema, and OPSS Schema would not allow the DBMS/Service, Hostname, or Schema Password to be modified on the JDBC Component Schema step during the Configuration.  All of the changes that were tried on these schemas kept reverting back to the previous generic values.
