The Latest Webgate Bundle Patch 11.1.2.3.180417 (BP10) is Breaking the Site Completely and Getting the Error: Bad Oracle Access Manager Request

Symptoms

Using OAM 11.1.2.3 and Webgate 11.1.2.3:

After applying the 11.1.2.3.180417 (BP10) Webgate (patch:27393427), it breake the site completely and the environment is down and getting the error below:

Then after applied the OAM latest Bundle patch, the site worked fine but after creating new use and redirect to the security questions page, the page is not displayed properly and the questions don't appear.
