Multiple Instances Of The Same Resource Created Via Access Policy For Same Account Discriminator

Goal
While trying to provision a resource, in some cases the mandatory values from the access policy do not propagate completely to the process form resulting in the provisioned resource going into "Provisioning" status. While the role and the corresponding access policy are evaluated again but it results in creation of another instance of the provisioned resource with the same data. We do not want it to keep creating instances unnecessarily.
As per the oracle document below multiple instances can be created via different values in Discriminator but this is not happening in our case:
