Running Upgrade Assistant in "-readiness" Mode Fails with Error "java.lang.NullPointerException" for Oracle Forms Services (Doc ID 2454699.1)

While trying to run upgrade assistant in readiness mode, for upgrading Forms and Reports 12.2.1.2.0 to 12.2.1.3.0, readiness check for "Oracle Forms Services" fails with "java.lang.NullPointerException".

Issue can be reproduced using below steps:
-----------------------------------------
1. Install and configure Forms and Reports 12.2.1.2..0 by changing Admin Server name to something other than default Admin Server (eg:admin)
2. Take a complete back up of 12.2.1.x.x Home
3. Install Fusion Middleware Infrastructure 12.2.1.3 in a new Oracle Home.
4. Install 12.2.1.3 Forms and Reports into the same Oracle Home as 12.2.1.3 Infrastructure.
5. Run upgrade assistant from 12.2.1.3 ORACLE_HOME in readiness mode as below
$ORACLE_HOME/oracle_common/upgrade/bin/ua -readiness
6. Readiness check will fail for "Oracle Forms Services"
