Disconnected Applications Not Getting Provisioned Any Further And No New "UPDATE UD_<Table>" Tasks Getting Generated For Provisioned Disconnected Apps

Goal
Suddenly Disconnected Application instances (supposed to be triggered through access policies or even direct provisioning too) are not getting inserted anymore for users as well as no "Update UD_<Table>" tasks getting generated for already provisioned Disconnected Apps. Also the same issue being seen for every other Disconnected Applications too so issue not specific to any particular Disconnected App.
Looking at ORCHPROCESS table for related entry shows value like following:
