Deployment Manager Not Working After 12c Upgrade - raising 'Private key is unavailable' Exception

After 12c upgrade, user attempts to open Deployment Manager (DM) from Identity Sysadmin console. The DM window opens, but none of the UI components render leaving the page blank and inoperable. In the diagnostic logs, the following exception is visible:
NOTE: This was reported in a 220.127.116.11 OIM standalone environment. The problem was in evidence only in the OIM domain. It is unclear if this could appear in an OIM-OAM integrated environment (shared domain) or what impact the issue might have on OAM.
Upgrade to OIM 12c.
