Deployment Manager Not Working After 12c Upgrade - raising 'Private key is unavailable' Exception (Doc ID 2459389.1)

Symptoms

After 12c upgrade, user attempts to open Deployment Manager (DM) from Identity Sysadmin console.  The DM window opens, but none of the UI components render leaving the page blank and inoperable.  In the diagnostic logs, the following exception is visible:

Upgrade to OIM 12c.
