OIM 12c Upgrade: Getting 404 errors with Garbled URL Redirect Containing Unresolved {app.context} When Accessing Log in to /identity or /sysadmin consoles (Doc ID 2459456.1)

Symptoms

When one attempts to connect to either /identity or /sysadmin consoles from browser:

   https://host:port/identity

Browser redirects to the following URL and raises a 404 error:

   https://host:port/{app.context}/adfAuthentication?success_url=/identity/faces/home?_adf.no-new-window-redirect=true

However, if the user enters a fully qualified URL like this, it actually displays the appropriate OIM login page:

   https://host:port/identity/faces/signin

Changes

Upgrade to 12c.  Additionally, tired to de-integrate OAM from OIM.  In 11g, OAM was set up to handle SSO (limited integration).   OAM was also installed in the same domain as OIM.
