Symptoms

1. Attempting to run the test form Form after a new 12c Forms and Reports new installation and an FRM-93552 occurs.

2. The formsap in WLS_FORMS is in a failed state.  When attempting to start it an error like the following occurs:

oracle.forms.servlet.ProcessManager.getHostStats()[J"

3. WLS_FORMS.log shows:

Error is: "java.lang.UnsatisfiedLinkError: oracle.forms.servlet.ProcessManager.getHostStats()[J"
