OIG 12c Upgrade: After Successful Bootstrap Completion, The OIM Consoles Are Not Accessible (Doc ID 2485833.1)

Symptoms

OIG 12c Upgrade: After successful bootstrap completion, servers are up but OIM consoles are not accessible. The following errors are seen in OIM log files.
