WebCenter Portal 11g to 12c Upgrade - Fails To Load Document Explorer

Symptoms
After upgrading the WebCenter Portal 11g (11.1.1.8.8) to 12.2.1.3, the Document Manager fails to load the taskflow with error:
"ADFC-06015: An exception occurred when invoking a task flow initializer.
The managed server log shows the below error:
