Symptoms

In the following scenario:

User1 has a manager as Manager1 and Manager1 has a manager as Manager2

User1 and Manager1 are active while Manager2 is disabled.

If one manager (not the immediate manager) has been disabled, when the SOA assigns the approval assignee, it runs into this error:
