On Oracle Access Manager 11g R2PS3 (OAM

OAM throws System Error while attempting to login to Forms via Webstart for a specific user

 "System error. Please re-try your action. If you continue to get this error, please contact the Administrator."

The issue can be reproduced at will with the following steps:
1. Login to Forms via Webstart
