12c Forms Managed Server Not Starting (Doc ID 2513223.1)

On : 12.2.1.3.0 version

12c WLS_FORMS server not starting

A new FMW 12c installation running the following components:

12c Forms and Reports
12c OAM
12c WCPortal
While performing some Forms Runtime LDAP Associations. After Associating the formsapp with LDAP host and LDAP port, an attempt to restart all the managed servers was made. All the servers came up (oam_server1, WC_Portal, WLS_REPORTS) except for WLS_FORMS.

We tried starting WLS_FORMS via the console numerous times but it fails to.  Bouncing the entire server did not help either.  Attempts to start just the WLS_FORMS server by itself never starts and just hangs.

In the node manager output following error is seen

The issue can be reproduced at will with the following steps:
1. associate formsapp/WLS_FORMS with an OID LDAP Host using Fusion MiddleWare EM per Document:

2. Restart all Managed Servers
