Accessing Oracle HTTP Server Home Page Fails With the Error "Error 404 --Not Found" (Doc ID 2573366.1)

Symptoms

After configuring SSL for Oracle HTTP Server which is part of Forms and Reports 12.2.1.3 installation, OHS home page cannot be accessed after starting WLS_Forms managed server.
Accessing Oracle HTTP Server home page shows below error:


Error 404 --Not Found

When WLS_Forms managed server is stopped OHS home page can be accessed.

During 404 error, checking certificates shows that demo certificate is being used though OHS is configured to use CA certificates.
