PDF Output From IRs Do Not Format Correctly After APEX Upgrade (Doc ID 2583699.1)

Symptoms

APEX has been upgraded from APEX 5.1.4 to APEX 19.1.

After the upgrade, PDF output from all interactive reports truncate columns. The columns are all formatted at the same width despite column size, so all columns do not display.
The problem can also be duplicated by creating a new IR, then changing alignment on some columns.

It has to do with format masks on columns. After adding format masks the APEX PDF generator doesn't format correctly.
- On the test page a new IR as a duplicate of the existing IR query.
- The report column order and column headings, and column alignments were changed. 
   -- PDF output was correct.
- Added format masks to the date and amount columns.
  -- Adding one at a time, each one would shift the PDF output.
  -- After 2 or 3 format masks, the columns would start to be truncated.

This problem has been duplicated with the ORDS print server as well as the external Apache FOP.
Have duplicated with ORDS listeners 18.1, 18.4, and 19.2
