Exalogic: ZFS upgrade from version 2013.1.5.4 to 2013.1.7.28 (April 2019 PSU) fails during patching (Doc ID 2599803.1)

Symptoms

Issues Upgrading ZFS appliance as part of patching from Exalogic July 2016 (2.0.6.2.160719) to April 2019 PSU (2.0.6.3.190416).

Output seen during patching task:

 

In the above log output the system will upload the files to the ZFS appliance, yet the upgrade will not occur leaving the current version. 

Furthermore the following can be seen via the ZFS BUI console, the current version running, and newer version with "exclamation" icon.  Clicking on the exclamation icon we see the following pop up window:
