Request for Application in SSL Says Certificate is Self-Signed and Insecure

Symptoms
When attempting to request the application deployed to WLS, using SSL the following error occurs as seen in FireFox.
It was also observed when viewing the certificate from the browser, that it is not using the expected signed certificate.
This was seen by comparing the Serial Number and Valid dates from the signed certificate with the one presented in the browser.
The issue can be reproduced at will with the following steps:
1. Request https://hostname.domain:
2. Receive the warning message in the browser showing that the certificate used is believed to be insecure and self-signed.
