Request Stuck In Status "Request Approved Fulfillment Pending" If Entitlement Removed Prior To Request Approval

If a Revoke Entitlement request is raised against a disconnected application instance but the entitlement is removed by other means (ex: API, Recon) before the request or associated manual fulfillment task is acted on then the request will be stuck in “Request Approved Fulfillment Pending”.
Since this is a disconnected application instance then the issue has two parts:
- request is approved and manual fulfillment task is raised (but not completed) prior to the entitlement being removed by other means
- request is not acted on at all (so no manual fulfillment task is raised) prior to the entitlement being removed by other means.
