"AttachToDocument, service is not initialized and reinit failed" Uploading Documents to WCI Knowledge Directory (Doc ID 2622978.1)

Symptoms

When attempting to upload a file into the WebCenter Interaction Knowledge Directory, the file can be selected, however, the upload fails with the following error:

Error - An error occurred during document import.

Looking at the error hidden in the source of the page or at a PTSpy log, the error is detailed:

-2147205117 - CPTSOAPProvider::AttachToDocument, service is not initialized and reinit failed.
