APEX 19.2 / SSO with OAM 12c Configuration Not Working - Page is not Redirecting Properly (Doc ID 2631124.1)

Symptoms

Note - 2457010.1 has been followed in order to configure Single Sign-on for an APEX application with OAM 12.2.1.3.
- Webgate is configured with OTD
  OTD is a software Load balancer that is available for deployment on Exalogic Engineered Systems.
  The link below provides steps for configuring webgate for OTD; the steps are almost exactly the same as those for OHS:

The appropriate header variables are being set properly; but the application, after login, goes into an infinite loop and then I finally get the error - Page is not redirecting properly.

In looking at the HAR output it can be seen that the APEX session ID is continually changing after the successful SSO login through OAM / Webgate:

Versions

APEX: 19.2.0.00.18
ORDS 19.2 is deployed on a Weblogic 12.2.1.3 Managed Server
OTD version is 12.2.1.3;
Webgate version is also 12.2.1.3

Other products, including OBIEE, Webcenter Content, ADF, SOA/BPM, etc, are configured with this set up and run successfully. The error is specific to APEX.
