OIM 12c Identity Console Not Accessible with HTTP 404 Error Due to Incorrect Application Configuration (Doc ID 2631298.1)

Symptoms

Oracle Identity Manager (OIM) Identity Self Service Console (http://<OIM_HOST>:<OIM_PORT>/identity) is not accessible with HTTP 404 error. The HTTP 404 error is displayed without the login screen. No errors are logged into any OIM log files when accessing the console.

Changes

In this particular case, the error started to occur after applying some patches to Fusion Middleware Home. But the patch related to the issue is not sure.
