Managed Server Health Shows "NOT REACHABLE" Status After Three Restarts Of Admin Server (Doc ID 2634922.1)

Goal

Managed Server health status will go to "NOT REACHABLE", after three consecutive restarts on Weblogic 12.1.3.

Follow the below steps to reproduce the issue:

1. Start the AdminServer (First time)
2. After the AdminServer started, use the AdminConsole to start the MS1 managed server
3. The MS1 (Managed Server) started with RUNNING and Health OK
4. Shutdown the AdminServer (First time - Shutdown)
5. Start the AdminServer (Second time - Start)
6. After the AdminServer started, the MS1 (Managed Server) status is RUNNING and Health OK
7. Shutdown the AdminServer (Second time - Shutdown)
8. Start the AdminServer (Third time - Start)
9. After the AdminServer started, the MS1 (Managed Server) status is RUNNING, but the Health is "NOT REACHABLE"
10. Shutdown the AdminServer (Third time - Shutdown)
