Unable to upload file into WebCenter Portal folder due to error "An item with specified name already exists" even though file does not exist in the folder (Doc ID 2651634.1)

Symptoms

In WebCenter Portal, user is unable to upload Readme.pdf into Folder2 due to error "An item with specified name already exists". 

However, there is no Readme.pdf in Folder2.

The following error was found in the UCM_server1-diagnostic.log after trying to upload Readme.pdf.

Also tried to upload ReadMe.pdf into another folder (e.g., Folder1) and move Readme.pdf from Folder1 into Folder2, but it failed with same error.

Tried to upload Readme.pdf into Folder2 using the CS native UI (i.e., http://<host>:16200/cs), but the file was renamed to "Copy of Readme.pdf" instead even though Readme.pdf doesn't exist in the folder.

Looked at FolderFiles table and see Readme.pdf in Folder2 along with "Copy of Readme.pdf".

In summary, the user was unable to upload Readme.pdf into Folder2, because there's a record in FolderFiles for Readme.pdf in Folder2 but it's not displayed in the application.
