Error "oracle.stellent.ridc.protocol.http.HttpProtocolException: Form validation failed" When Attempting To Release A Document Directly From Capture to UCM (Doc ID 2659457.1)

Symptoms

Unable to release a document successfully using WebCenter Enterprise Capture.

Created a workspace in WebCenter Enterprise Capture 12.2.1.3, and created a client profile with a UCM Commit profile.

The connection to UCM or WebCenter Content is working well, however when releasing the document in the Capture client,  the document is not committed to WebCenter Content and the following error appears in the Capture log file:

oracle.stellent.ridc.protocol.http.HttpProtocolException: Form validation failed
at oracle.stellent.ridc.protocol.http.auth.FormAuthHandler.throwFormValidateException(FormAuthHandler.java:387)
at oracle.stellent.ridc.protocol.http.auth.FormAuthHandler.handleFormResponse(FormAuthHandler.java:364)
at oracle.stellent.ridc.protocol.http.auth.FormAuthHandler.handleFormLogin(FormAuthHandler.java:160)
at oracle.stellent.ridc.protocol.http.auth.FormAuthHandler.sendAuthenticatedRequest(FormAuthHandler.java:122)
