OIM 12c: /images/info.png Image Can Not be Used to Display

Symptoms
The /images/info.png that is Oracle Identity Manager (OIM) out-of-the-box icon image can not be used to display.
The info.png is one of infomation (i) icon image. For example, it's used for the password policy tips on the provisioned account. We can change the password information icon on account form as follows:
OIM Identity Console -> Create a sandbox -> Open Users -> Search and open an OIM user -> Account tab -> Select an account to open details -> Click Customize link -> Open Structure tab -> Select Password icon image (UI component name: commandImageLink) -> Edit it to open component properties -> Change Icon field to /images/info.png -> Click OK
The info.png can not be displayed even also accessing its URL directly;
http://<OIM_HOST>:<OIM_PORT>/identity/images/info.png
http://<OIM_HOST>:<OIM_PORT>/sysadmin/images/info.png
Other information icons can be displayed, for example;
http://<OIM_HOST>:<OIM_PORT>/identity/images/info_ena.png
http://<OIM_HOST>:<OIM_PORT>/identity/images/info_dwn.png
Changes
The issue occurs in both clean installation and upgraded installation.
