Content Integration Does Not Work in WebCenter Portal : The Documents Page Shows Blank and Attempting to Add the Content Manager Task Flow to a Page Does Nothing

Symptoms
Content Integration Stops Working in WebCenter Portal After Upgrading from 12.2.1.3 to 12.2.1.4.
The following issues are observed:
- The Documents Page Shows Blank.
- Attempting to Add the Content Manager Task Flow to a Page Does Nothing.
- Enabling the Documents Service in a Portal creates the folder in Content Server but content can't be accessed from WebCenter Portal.
ERRORS
- Enabling documents service in a portal causes the following errors in the UCM_server1-diagnostic log:

STEPS
The issue can be reproduced at will with the following steps:
- Connect to WebCenter Portal.
- Enable the Documents service in the portal.
- Check the UCM_server diagnostic log.
Here you will see the error. - Add a page link to the documents page in the portal.
- View the portal and navigate to the Documents Page.
Here you will see a blank page. - Edit the portal.
- Create a page.
- Add the Content Manager Task Flow.
Here you will see the task flow is not added to the page.
Nothing happens when you try to add the task flow.
Changes
WebCenter Portal was upgraded from 12.2.1.3 to 12.2.1.4.
