Symptoms

In cluster environment, sometimes following error output in the diagnostic log

Internal Exception: java.sql.SQLIntegrityConstraintViolationException: ORA-00001: unique constraint (XXX_SOAINFRA.BPM_MEASUREMENT_ACTIONS_U1) violated

Error Code: 1
