Login Using Oracle HTTP Server to WebLogic Admin Console Fails with "Authentication Denied" (Doc ID 2717310.1)

Symptoms

When trying to login to WebLogic console using OHS, WLS Admin Console fails with "Authentication Denied" and "The username or password has been refused by WebLogic Server" as shown in the screen below:

The OHS log shows no error message.

 But able to login and access EM, Analytics, xmlpserver pages via Oracle HTTP Server.
