Applying WebLogic PSU Patch Fails with the Error "Installed patch does not have catalog entry"

Information in this document applies to any platform.
When trying to apply the latest PSU patch, it shows the conflict message "Conflict(s) detected - resolve conflict condition and execute patch installation again".
Trying to remove the conflicting patch fails with the message "Encountered unrecognized patch ID".
Running the bsu report shows no patches applied to Weblogic though there is a conflict.
The log file shows the below message:
