UCP Connections Fail With "Exception occurred while enabling failover with remote ONS subscription" (Doc ID 2729059.1)

Symptoms

After upgrading an application to use version 19.3 of UCP, database connections are failing with the following error:

java.sql.SQLException: Could not create datasource
...
Caused by: java.sql.SQLException: Exception occurred while getting connection: oracle.ucp.UniversalConnectionPoolException: Exception occurred while enabling failover with remote ONS subscription
at oracle.ucp.util.UCPErrorHandler.newSQLException(UCPErrorHandler.java:456)
at oracle.ucp.util.UCPErrorHandler.throwSQLException(UCPErrorHandler.java:133)

Caused by: oracle.ucp.UniversalConnectionPoolException: Exception occurred while enabling failover with remote ONS subscription
