Oracle Access Manager (OAM) Forgot Password flow With One-Time Password (OTP) getting HTTP 500 (Doc ID 2751421.1)

Symptoms

Using OAM 12.2.1.3 and Forgot Password With One-Time Password is not working. While trying to generate OTP for password reset getting HTTP 500 Internal Server Error using bellow steps:

1. On login screen click on "Forgot Password"
2. Input username and click on "Generate PIN"
3. HTTP 500 error seen on /otpfp/initChangePswd OAM endpoint
