Installation of a PSU Completes Successfully but it is not Listed by: opatch lsinventory

Symptoms
On : 12.2.1.3.0 version
PSU not found in opatch lsinventory output after it was applied without any error.
Applied SPB Patch April 2021, <Patch 32755804> and showed every thing was success during patch application.
After this the "opatch lsinventory" command did not report any PSU applied.
On the log file it is noticed the following log-entries:
(Notice four patches included in the SPB, including 32697734 which is PSU for 12.2.1.3.210329)
---> Notice also the line for each of the patches that read: The actions are reported here, but are not performed.
