Tuxedo Fails to Restart With Duplicate Server Warning

Symptoms
When a Tuxedo server process (CORBA server) was shutdown with a -k TERM option, the process did not register as stopped (think it really did but state did not update)
tmshutdown -k TERM -g GRP1 -i 1080
A subsequent (25 seconds later) tmshutdown -w 1 also failed to stop the process... (GRP1/1080 process)
tmshutdown -w 1 -g GRP1 -i 1080
with the error message tmshutdown: WARN: internal error: CMDTUX_CAT:943: INFO: Server (UC_GRP1/1080) was shutdown for migration
But when attempting to restart the process would not start with 'duplicate server' warning, yet it was not running. tmadmin bbclean and pclean did not make a difference.
The only way to clear this bad state was to reboot the server and then bring it back into the tuxedo domain.
