Exalogic Virtual: Compute Node Fails to Boot Up Following Reboot Due to Incorrect UUID Entries in /etc/fstab (Doc ID 2817600.1)

Symptoms

On racks upgraded from 2.0.6.3.x to 2.0.6.4.0 using the Upgrade Kit, one or more compute nodes fail to boot up following a reboot. This is typically encountered while applying a PSU to the upgraded rack but may also be encountered during a planned or unplanned reboot. The following UUID related error is observed in the ILOM console:

The Exalogic Rack was recently upgraded from 2.0.6.3.x to 2.0.6.4.0 using the 2.0.6.4.0 "Upgrade Kit".
