Symptoms
Running any form twice consecutively results in the FRM-93618 error. The form runs without error the first time but always fails on the second attempt.
Issue reproduces using any form including the test.fmx.
Issue reproduces when running from the Builder and also from Runtime.
The WLS_FORMS managed server needs to be restarted to resolve the issue.
