Getting Error "OTD-67755 An error occurred while reconfiguring the server" When Activating Changes from Fusion Middleware Console (Doc ID 2825853.1)

Symptoms

When activating changes made from /em console the following error occurs in the Admin Server log. The node manager records the same information about the failure in its log. But nothing is logged to the the OTD server log.
