Running the Content Upgrade Assistant Command 'ua -readiness' Fails with 'TEST_UNEXPECTED_INDEXES' (Doc ID 2853023.1)

Symptoms

During an Imaging upgrade from 11g to 12.2.1.4, in the "ua -readiness step", the following errors occur:
