Requests to Application Intermittently Fail with Return Code 404 (Doc ID 2856860.1)

Symptoms

Application was not working 100% OK because intermittently after having success processing some requests, within a few minutes or seconds  same requests submitted failed with HTTP return code 404 [Not Found].
