Upgrading from 11.1.1.8 to 12.2.1.4 Generates Various OPSS Errors (Doc ID 2880744.1)

Symptoms

When upgrading from 11.1.1.8 to 12.2.1.4, OPSS errors are occurring.

First set of symptoms:

After the new domain was created using the template from the modified OLD domain template, the upgrade to 11.1.1.9 occurred without error.  The customer then started implementing the 12.2.1.4 upgrade. 

While running the 12c Reconfiguration Wizard, reconfig.sh, the error occurred:

oracle.security.jps.upgrade.tools.utility.Upgrade - Upgrade DIT and Data error!
oracle.security.jps.JpsException: JPS-00031: Cannot load the required class oracle.security.wls.oam.providers.sso.OAMSSOServiceProviderImpl. Reason java.lang.ClassNotFoundException: oracle.security.wls.oam.providers.sso.OAMSSOServiceProviderImpl.
