Symptoms

An additional “<env:Header/>” tag started appearing in the response of a custom JAX-WS Web Service before starting of "<S:Body>" tag.
Issue started happening after WebLogic upgraded from 12.1.2.0 to 12.2.1.4.0.

Changes

 Upgrade WebLogic from 12.1.2.0 to 12.2.1.4.0.
