Forms Related Actions Like Running the Test Form via FSAL, Lead to Error 404 (Doc ID 2891717.1)

On a fresh install of Oracle Forms, the following error is being encountered when attempting to run the test form via Form Standalone (FSAL). Similar errors are also encountered when performing Forms related actions like downloading fsal.jar. The issues occur even when the Node Manager, Weblogic server and WLS_FORMS are all up and running.

 Observation: The deployment formsapp(12.2.1) (Enterprise Manager-> Deployments) never has a State of 'Active'. The values that are seen are either 'Installed' or 'start Initializing' when there was an attempt is to restart the deployment.

FRM-92497: Server returned HTTP response code 404--Not Found.

The actions below also cause the Error 404 to be raised:

a. downloading frmsal.jar, e.g. http://<hostname>:<port>/forms/html/fsal.htm

b. accessing the urls http://<hostname>:<port>/forms or http://<domain>:<port>/forms/frmservlet

Steps to reproduce:
The issue can be reproduced at will with the following steps:

1. log on to the server

2. open a Terminal window in Unix or a Command Prompt in Windows.

3. copy the file $ORACLE_HOME/forms/java/frmsal.jar into a valid directory.

4. issue the command below via the same directory where the above file was copied into:

   java -jar frmsal.jar -url " http://<hostname>:<port>/forms/frmservlet?config=standaloneapp"
