Unable To Add A New Metadata Field To An Imaging Application Definition Field (Doc ID 2902169.1)

Symptoms

When trying to add a field to a specific application, it fails with the below error.  Adding fields to other applications works fine.  

The Following Error was reported by the Repository: Cannot modify Application, Unable to add new Meta Definition field X.IPM_APP_XX_XX. Meta Definition field X.IPM_APP_XX_XX Already defined
